5/10/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Jashim upon arrival to the home setting. Upon arrival to the home setting, Jashim appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C inquired about his behaviors since the last session. Mother reported that he continues to struggle with being able to control his emotions. 2. Behavior: Jashim presented off task behaviors as he was looking at this brother play the game. Once he was redirected he complied with the remainder of the session. 1.Intervention: QMHP-C informed Jashim that session would focus on learning coping skills to reduce anger to continue to build off what has already been reviewed for the week. QMHP-C instructed Jashim to address current behaviors in the home and community and identified his triggers and fuses that causes his anger and frustrations. 2. Intervention: QMHP-C provided Jashim with respectful ways to communicate and respond to his mother and other authority figure without anger and frustrations. QMHP-C used modeling techniques to illustrate the correct way to appropriately converse with an adult and his peers when resolving conflict. QMHP-C reviewed calming techniques with Jashim to utilize when struggling with aggressive behaviors toward peers. 1. Response: Jashim appeared to be receptive to QMHP-Csâ interventions and services that were provided to him by counselor. Jashim was able to verbalize how he can change negative behaviors into positive behaviors. Jashim was able to indicated acceptable and unacceptable behaviors. Jashim reported that he struggles with responding negatively when others disrespect him. 2. Response: Jashim reported that he is easily triggered and angered by his peers when they constantly go back and forth with his friends that cannot fight or defend for themselves. Jashim admitted to counselor that is his weakness and mostly the reason as to why he gets in trouble in the community and when he was in school because he donât like people
